Adult Social Work Team
The Adult Social Work Team is based in the Council Offices at the Patterson Kirk on School Place, telephone 01856873535 and ask for Adult Social Work Team.
There is a duty worker available from 09:00 to 13:00, Monday to Friday to answer enquiries by phone or in person. In an emergency, a duty worker can be contacted between 13:00 and 17:00 Monday to Friday.
Duty workers have access to considerable information and local knowledge and are happy to give advice about a range of issues or signpost people to other organisations who may be able to help.
The team can accept referrals to provide an assessment where a person’s disability or illness means that they need help to manage any aspect of day to day life. Assessments may lead to services such as home care; day services; residential care or respite breaks being offered or a direct payment so that the person or their representative can arrange their care. Social workers make assessments in co-operation with an individual and their carers along with other professionals and organisations who may be involved.
The team work with people living at home, in residential care or supported housing or who are about to be discharged from hospital. We offer our social work skills to help people and their carers make essential changes to their lifestyles, whatever the cause.
We give advice and guidance in relation to the Adults with Incapacity Act, where people need to use the act to plan for the future or where guardianship powers may need to be sought.
The team take referrals where an adult may be at risk of harm and may use the Adult Support and Protection Act to ensure an individual is assisted to be safe.
The team can make assessments about providing emergency financial or other support where an individual has no other source of help.
